Transitional Living Program

NFHC’s Peaks Transitional Living Program is operated through a federal funded grant issued by the Family Youth Services Bureau Runaway and Homeless Youth Program. Peaks Transitional Living Program (TLP) provides housing, skills training, and support services to youth ages 18-21, or emancipated youth ages 16 and 17, on a first come/first serve basis for 12-18 months. The program is voluntary and there is no cost to participate. Participants in the program are expected to follow program rules and guidelines and make consistent effort toward their goals as set forth in their individualized service plans in the areas of employment/vocation, independent living skills, education, and mental and physical health. Services will be provided by Northland Family Help Center staff, including case management, educational and life skills groups, transportation assistance, counseling services, and additional referrals to community-based agencies and organizations.
